Dialysis Management
Coordinated care for patients on hemodialysis or peritoneal dialysis, including access management, adequacy monitoring, and transition support.
What is Dialysis Management?
Dialysis replaces the kidneys' filtering function when they can no longer do so on their own. Our team manages all aspects of dialysis care — from choosing the right modality to long-term monitoring — ensuring patients receive safe, effective treatment with as little disruption to daily life as possible.
